Other Important Information:

1. LVT Corp. does not provide a firewall for your computer(s). The High Speed Internet Service is "always on", so your computer is open to the outside world. Customer is responsible for providing their own firewall.
2. These prices do not include telephone service. Telephone service is required for internet service.
   
3. Sales Tax is not included in the prices stated above.
4. Subsequent visits to the customer's premises will incur a trip charge ($25.00) and labor charges ($40.00/hour).
5. Static IP Addresses - $5.00 per IP Address per month.
6. Separate fees for material such as wiring and telephone jacks may be charged to the customer's account.
7. Install fees may apply
   
8. An email account is included with internet service.

Billing information and other fees

1. Billing begins the day the service is activated. Your first bill may reflect prorated service and one full month of service.
2. Reconnection of service for non-payment - $12.50
3. Some account changes may result in additional service fees.
