Other Important Information:
- Customer needs Ethernet or USB port on their computer. (Windows 95,98, 2000, ME, XP or NT 4.0) Customer is responsible for this equipment.
- LVT Corp. does not provide a firewall for your computer(s). The DSL service is "always on", so your computer is open to the outside world. Customer is responsible for providing their own firewall.
- These prices do not include telephone service.
- Sales Tax is not included in the prices stated above.
- When purchasing a DSL modem from LVT Corp, customer is provided a 30 day warranty from LVT Corp. After LVT Corp 30 day warranty expires, the manufacturer warranty is effective.
- Subsequent visits to the customer's premises will incur a trip charge ($25.00) and labor charges ($40.00/hour).
- Static IP Addresses - $5.00 per IP Address per month.
- Separate fees for material such as wiring and telephone jacks may be charged to the customer's account.
Billing information and other fees
- Billing begins 30 days from the day the service is activated. Your first bill may reflect prorated service and one full month of service.
- Reconnection of service for non-payment - $11
- Service/record change - $11
